Sorry, should have gone back around one more time. I have this working now. This issue is resolved. Issue was resolved by copying the correct default folder from geo-my-wp (NOT global add-on!), and then cycling the them back to default and then to the new custom form template.
Issue is resolved!
Progress–I think I was using the wrong default folder. But now when I choose it, the form doesn’t appear at all above the map. Again, I haven’t customized search-form.php or its CSS in any way in the folder.

Thanks for all your work on the plugin, Eyal. Any update on this? I too need a map tool that can have drop-downs for taxonomoy/custom fields. Sounds like this still isn’t possible?
Can anyone verify whether this has been added to the Premium Settings add-on yet?

So in general it just seems like the search doesn’t really work.
– Keywords don’t find any results no matter what you put in
– If you search an address, the results page doesn’t actually show the results…it just continues to show all results.
– I got rid of the state drop-down because it wasn’t working either
In general, it just seems like the search function isn’t actually searching the custom post type in any way except for a full address. If you enter “Texas” into the address field, it finds nothing. But if you enter San Antonio, Texas, it does find some elements. Does it have to be at least drilled down to the city?
Additionally, the plugin still has multiple issues whenever I turn BuddyPress on. Any leads there? The only difference in the page is that it doesn’t have a sidebar and the default page does.
